Spontaneous idiopathic chylothorax in adults.

چکیده
hylothorax refers to the presence of chyle in the pleural cavity as a result of thoracic duct disruption. It is either spontaneous or traumatic. Typically, it appears as a milky, opalescent odorless fluid, rich in triglycerides. Herein, 2 cases of spontaneous idiopathic chylothorax are reported, with a brief review of the literature. A 37-year-old Saudi man, who was admitted for investigation of a left basal opacity that was detected during medical fitness screening. He reported nocturnal episodes of dry cough and dyspnea with spontaneous resolution, however, he denied any medical treatment. On examination vital signs were within normal limits. There were old cautery scars over the right upper quadrant of the abdomen. Chest examination demonstrated signs of a left-sided pleural effusion. Chest radiography showed a left basal homogenous opacity suggestive of pleural effusion and right upper lobe linear shadow suggestive of azygus fissures (Figure 1). Blood investigations including complete blood count (CBC), erythrocyte sedimentation rate (ESR), liver and renal function tests were all within normal limits. Thoracentesis yielded 800 ml of a pinkish milky fluid, its analysis confirmed the presence of chyle with high triglycerides and low cholesterol level (Table 1). Cytology of pleural fluid revealed lymphocyte predominance, negative for malignant cells. Mantoux test was carried out and direct smear and culture for acid fast bacilli were negative. Spontaneous Metachronous Chylothorax—Presenting As a Neck Lump
Chylothorax is defined as an accumulation of chylecontaining lymphatic fluid within the pleural space. Most adult chylothoraces are related to surgery or malignancy. Spontaneous idiopathic chylothorax in adults is an extremely rare occurrence with very few reported cases. We present the first described case of spontaneous metachronous chylothorax presenting as a neck lump in a 48-year-old Cauca...

Background Chylothorax is defined as abnormal accumulation of lymphatic fluid in the pleural space secondary to leakage from thoracic duct or one of its main tributaries. Congenital idiopathic chylothorax is the most common form of pleural effusion in neonates. Conservative therapy is the mainstay of treatment. An alternative...
متن کاملOctreotide for the Management of Chylothorax in newborns, case report
Chylothorax is the most common cause of pleural effusion in neonates. It is usually idiopathic. Neonatal chylothorax successfully respond to octreotide treatment and can reduce the duration of hospitalization. A number of therapeutic interventions have been used to reduce chyle production and promote resolution of a chylothorax. Initial management typically includes restriction or temporary ces...

Eighteen patients who had developed a chylothorax during a 25 year period, 1955-80, were reviewed. The chylothoraces occurred as a complication of cardiothoracic surgery in 11 patients, of whom eight were children in the first decade of life. Five cases followed operations for coarctation of the aorta and two occurred as a complication of a Blalock shunt for Fallot's tetralogy. The chylothorax ...
متن کاملSpontaneous chylothorax associated with light physical activity.
Chylothorax occurs when there is rupture, laceration or obstruction of the thoracic duct, resulting in the release of chyle into the pleural space. Chylothorax can occur in cases of congenital lymphatic malformation, lymphoma, mediastinal tumor and infectious disease, as well as during surgical procedures and after traffic accident-related trauma. It can also be idiopathic. The condition presen...
